New C-2 Employer's Report of Work-Related Injury/Illness must be in use by January 1, 2009

As part of the Workers' Compensation Reform initiative, the NYS Workers' Compensation Board has redesigned several key forms the Board uses to gather information about a claim. One of these is the C-2 Employer's Report of Work-Related Injury/Illness.

This form now requests additional information, such as:

  • Workers' Compensation policy period

  • Date the employee gave notice of accident/illness

  • Employee's supervisor and a list of any witnesses

As always, we encourage you to fill out the form as completely as possible and to submit it to PRM Claim Services, Inc. immediately following a work-related injury.

A sample copy of the new C-2 form is available by clicking here. Soon this form will be included in our Online C-2 claim reporting module, and our WebClaim Online Claim Reporting System is currently under revision to accommodate the new form as well.

All parties are to begin using the new C-2 forms as soon as possible, but no later than January 1, 2009. In the interim, the forms in use on our website and our WebClaim system may still be used until the newly revised forms are in place.
